Eileen Tokuda, PT, MHA

Eileen Tokuda, owner and physical therapist at Healing Motion Physical Therapy in Philomath, Oregon.Eileen established Healing Motion Physical Therapy, Inc. in 2005.

She obtained her Physical Therapy degree at the University of Pennsylvania and practiced in a variety of clinical settings. This included acute hospital care (with concentration on post surgical care), rehabilitation centers (with emphasis on spinal cord injury rehabilitation), home care (inclusive of hospice services) and outpatient orthopedic care for over 15 years.

She then obtained a Masters of Health Administration degree from the University of Washington and set strategic direction for health education classes, managed implementation of health promotion programs, and led start up of a company that established operating systems for assisted living and retirement community operations.

She is also the proud parent of two young men, identical twin boys, who are now attending Oregon colleges.

She is joined in her work at Healing Motion PT by her husband, Craig, who has been instrumental in establishment of the accounting and billing systems that ensure compliance with insurance billing requirements.

In her “free” time, Eileen is an avid bicyclist, enjoys skiing and snowshoeing, enjoys her sons’ lacrosse teams, and relaxes with a favorite glass of wine and a good book of historical fiction!

Ashlee Roderick, PT, DPT, OCS

Ashlee joined HMPT in June of 2015. She graduated from Corban University magna cum laude in 2011, where she majored in Health Science, Human Performance with an emphasis on Exercises Science, and minored in Biblical Studies. She played basketball on a scholarship for all four undergraduate years and was an Academic All-American and All-Conference player. Prior to her physical therapy training, Ashlee worked in various facilities, including our local Fitness Over Fifty, as a personal trainer.
Ashlee attended graduate school at Pacific University and graduated Doctor of Physical Therapy with distinction in May of 2015. She achieved her Orthopedic Clinical Specialist certification in 2018 and is pursuing additional specialty certification in orthopedics and manual therapy through the North American Institute of Orthopedic Manual Therapy. Ashlee particularly enjoys treating athletes. She is one of few local pelvic floor specialists. She also has a sizeable interest in pain neurophysiology as it relates to chronic pain. Additionally, she is a primary investigator in “The Effectiveness of Astym® in Treating Plantar Fasciopathy: a feasibility study.”
Ashlee, her husband, and their little girl make their home in Albany. They spend their time together with friends and family, running, playing basketball and disc golf, wakeboarding, and cheering on the Blazers, Red Sox, 49ers, and of course the BEAVS

Lisa Koslicki PT, DPT

Lisa received her Doctor of Physical Therapy degree from George Fox University in Newberg, OR. Hailing from the beautiful Oregon coast, Lisa graduated from Eastern Oregon University cum laude in 2010 with her degree in Physical Activity and Health with a Health and Wellness concentration. Afterward, she worked as an administrative assistant at an outpatient physical therapy clinic, where she added a working knowledge of clinic procedures to her passion of serving patients therapeutically. During Lisa’s graduate internships in outpatient clinics, skilled nursing facilities, and hospitals, she treated patients across the continuum of injury and disease progressions. Through her experience, she developed a growing interest in manual and soft tissue therapy techniques and plans to continue honing these skills. Lisa is committed to providing quality patient care as part of the dedicated HMPT team, and is always on the lookout for new research and techniques to better assist her patients in returning to their highest level of function.

Lisa lives in Corvallis with her husband, where they enjoy their sweet dog and adventuring in the outdoors together. Outside of work Lisa loves to cook, play music, craft cards, plant flowers in the sunshine, and explore new scientific discoveries.

Kersey Schuh, PT, DPT

Kersey, a Corvallis native, graduated from Oregon State University cum laude in 2013 with a degree in General Science with a concentration of Pre-Physical Therapy. She attended Emory University in Atlanta, Georgia for graduate school and received her Doctorate of Physical Therapy in May 2017. In her clinical internships in outpatient orthopedics, pediatrics, and hospital acute care she was able to experience treating a variety of injuries and disease processes, with people from 4 months to 96 years of age. During her orthopedic internship she was introduced to manual therapy techniques and has a desire to continue to build upon those skills. Her interests lie in post-surgical knees and hips, low back pain, and to continue to develop further knowledge and skills to treat throwing-related shoulder injuries. Kersey is excited to be a part of the HMPT team and to assist people to live their best life through building relationships with each patient and furthering her skills as a clinician.

Outside of work Kersey enjoys spending time with family and friends, playing with her adorable puppy, reading, baking, and decorating cakes. She is also an avid baseball fan – Go Braves!

Elizabeth Hadachek, PT Assistant

Elizabeth graduated from the University of New Orleans in 1992 with a Bachelor of Arts in English. While working on her master’s in education, Elizabeth was introduced to physical therapy and decided to volunteer at Ochsner Foundation Hospital in rehabilitation services. As a result of that experience, she was inspired to change her path and pursued a career in physical therapy. She graduated from Louisiana State University/Delgado Community College as a Physical Therapist Assistant in 1998. With 20 years of experience, Elizabeth comes to HMPT with an extensive clinical background. She has worked in a variety of settings including hospital acute care, oncology and bone marrow transplant units, in-patient rehab units, out-patient orthopedics and pediatrics, and home health. She has a passion for learning new therapeutic skills and treatment techniques to continue her growth as a clinician and to better assist her patients’ recovery. She truly enjoys the collaborative approach to patient care and works closely with her colleagues to help patients achieve the best possible outcomes.
Elizabeth and her husband live with their daughter in Corvallis. They enjoy outdoor adventures with family and friends. Elizabeth loves cooking, running, hiking, yoga, and exploring the Pacific Northwest.

Kelsey Huntington, Office Coordinator

Kelsey recently graduated with a Bachelors Degree in Kinesiology from Oregon State University and is planning to attend graduate school for Physical Therapy in the next year. She is originally from Salem, Oregon and enjoys hiking, cheering on the Beavs and Chicago Cubs, and spending time with friends and family.

Tiara Stahlbaum, Physical Therapy Aide

Tiara is originally from Oregon, but grew up moving around the U.S., Germany, and Spain. As a soon-to-be Kinesiology graduate from Oregon State University, she plans to attend graduate school to pursue a career in Physical Therapy and/or Public Health Promotion. In her free-time, she enjoys equestrian dressage and eventing, hiking the beautiful PNW with her Golden Retriever, cooking, drawing, and graphic design.

Kyla Baternik, Accounting Assistant

Kyla grew up in the suburbs of Boston. After high school, she moved to Ashland, Oregon where she graduated from Southern Oregon University with a Bachelors and Masters Degree in Accounting. Before settling down in Oregon, she lived in Salt Lake City, Utah, and Alburquerque, New Mexico.
Kyla now resides in Philomath, Oregon with her husband and son. She enjoys spending time with her family hiking, running, reading, cooking, crocheting, and training her dog for agility competitions.

Craig Tokuda, Accounts Receivable

Craig Tokuda, billing administrator at Healing Motion Physical Therapy in Philomath, Oregon.Greeting you in his warm Hawaiian style, Craig is our chief Billing/Accounting Administrator. After a long career in finance Craig retired from Hewlett Packard and has been instrumental in helping Eileen establish Healing Motion PT. In Craig’s free time, he enjoys bow hunting.
